OFFICE HOURS:

Monday through Friday
7:30am to 6:00 pm
Closed Saturday and Sunday

NEW PATIENTS:  Please arrive 15 minutes early for your first appointment to complete the necessary paperwork.  All initial paperwork can be downloadedand filled out in advance.

RETURNING PATIENTS:  Please arrive 5 to 10 minutes early to update necessary paperwork before your visit. 

We are now located in at 1441 SW Chandler Avenue, Suite 103.  This is off of the Century/Chandler/Columbia traffic circle.  We are in the building directly next door to the Cascade Lakes Lodge Restaurant/Brewery.
